High Serum Ferritin in Adult-Onset Still's Disease  [PDF]

Abstract: In August 2009, a 28-year-old Spanish woman was referred in reduced general condition after at least four medical consultations with a 2 week history of remittent fever, night sweats, exanthema and intermittent arthralgias. Noticeable in the clinical examination was only an urticarial rash on the trunk and extremities and body temperature of 39.6°C. Laboratory tests: WBC with a slight left shift (32%), mild thrombocytopenia (128 G/l), CRP 78 mg/l (N: < 5), elevated transaminases [ASAT 67 U/l (N: 10-37), ALAT 45 U/l (N: 30-65)]. On the fifth day maximum values for ASAT 322 U/l and ALAT 378 U/l were reached. During hospitalisation recurrent fever up to 39.0°C has been documented associated with arthralgias and myalgias. An infection with blood cultures and serology tests was excluded. Rheumatology analysis, urine analysis, chest X-ray, and abdominal ultrasound were inconspicuous. A serum ferritin level of 5493 μg/l (N: 23-110) was detected with peak four days later (8530 μg/l). AOSD was suspected and prednisolone (1 mg/kg body weight) started. The patients recovered rapidly. Transaminases (ASAT 47 U/l, ALAT 149 U/l), CRP (34 mg/l), and ferritin level (1969 μg/l) regressed within 3 days. 3 months later, prednisolone was discontinued; she presented fever (40°C), myalgias, macular rash on the trunk, sore throat and oligoarthritis. Laboratory tests including WBC were without pathologic findings except for transaminases (ASAT 83 U/l, ALAT 53 U/l), CRP (370 mg/l) and ferritin (7434 μg/l). An infectious process was excluded. After resumption of prednisolone 10 mg daily, quick relief of symptoms occurred. Given the short time of relapse, immunosuppressive therapy with cyclosporin was initiated (4 mg/kg body weight). Since that time the patient remained symptom free for over 8 months.

Abstract: the objective of this research was to evaluate, in laboratory, the development of lutzomyia intermedia and lutzomyia longipalpis (diptera: psychodidae: phlebotominae) larvae, vectors of leishmaniasis in brazil, in the following diets: industrialized food for rabbits, dogs, hamsters and aquarium fishes, besides liver powder, cooked lettuce, wheat germ, beer yeast, oat, wheat bran and a diet denominated aged food. except wheat bran for l. intermedia, all diets provided adequate development for both species, which showed that any of them can be used in laboratory insectaries for these insects. l. intermedia showed better development with most nutritious diets and both species presented better development with aged food. fungi as an additional nutrient source for l. intermedia and l. longipalpis is suggested.

Abstract: this paper argues the historical process of professional education models construction in the brazilian high schools to help the understanding of this education modality questions. it approaches the high schools ambiguity, the association between social discrimination and technical occupations, containment of demand to the universities, citizen's integral formation and formation to the work, pointing out the health area in this context. it had been used, as sources, the brazilian legislation for the education and other governmental technical documents. the article intends to show the importance that the finish point concept acquired in the professional education proposals, pointing the politecnia as alternative to the school and professional formation, designating the peculiarities of the health area.

Abstract: the automotive industry is undergoing transformations that have led to the segmentation and diversification of its market, reductions in the life cycle of vehicle models, and the offer of a variety of features of these models to consumers. within this context, this paper analyzes these trends and evaluates the key strategies developed and implemented by brazilian and european automakers in face of these trends. our findings, which are analyzed using the same methodology adopted in a similar survey conducted in europe, indicate how platform, late configuration and option bundling strategies have been implemented by automakers, allowing for a comparison of the two markets in question.

Abstract: there is a discrepancy between the demands of the labor market in the health sector and professional technical colleges. the quest for comprehension of one of the aspects pertaining to the distance between professional formation of technicians in clinical analyses and the necessities of the labor market is the focus of this research. the general purpose of this study is to understand the teaching methodology used by professors in biosecurity at technical colleges that offer courses in clinical analyses, in conjunction with a high school in the metropolitan region of rio de janeiro, where data was collected from public teaching units of the state of rio de janeiro. when dealing with a case study with a qualitative approach, information was obtained by means of recorded interviews that were guided by a semi-structured questionnaire, whose content was analyzed by three biosecurity professors. the results of the survey indicate the necessity for reformulations in pedagogic practice, enabling the formation of technicians in clinical analyses to be more in line with the actual necessities of the private or public market represented by the public health system (sus).

The 55-year-old wife complained in June 2010 about pain in her neck. Case history, physical examination, and erythrocyte sedimentation rate (ESR) of 80 mm/hour led to the diagnosis of PMR. In May 2011, her 66-year old husband complained about pain in his neck, shoulders, buttocks, and thighs. Considering anamnesis, physical examination, and ESR of 56 mm/hour, the diagnosis of PMR was made. Both wife and husband responded to steroid treatment. When the steroid dose was gradually reduced, both patients relapsed. In order to lower the cumulative dose of glucocorticoid therapy, 10 mg methotrexate per week was added. In the literature, six cases of polymyalgia rheumatica in married couples have been described to date. In four cases, polymyalgia rheumatica occurred first in the wife. The interval of the diagnosis between the spouses ranged from 0 to 89 months. Although in most of the previous case reports a genetic disposition and an infectious agent have been discussed, this hypothesis must be questioned.

Abstract: As pequenas e médias empresas (PMEs) vêm assumindo importancia crescente na economia mundial, em um cenário de press es para uma competi o global. Este trabalho tem como objetivo analisar o uso de networks no processo de internacionaliza o de PMEs. Para tanto, foi realizada uma revis o da literatura publicada sobre networks, buscando-se analisar os cenários de aplica o, as motiva es para a utiliza o dessa estratégia e suas vantagens e desvantagens. Foram consolidadas as informa es secundárias de casos publicados sobre PMEs latino-americanas que utilizaram networks para a entrada em mercados internacionais. Como resultado, foram encontrados indícios do uso de networks na internacionaliza o de PMEs, podendo gerar vantagens e desvantagens para as empresas envolvidas. Também foi observada a relevancia do papel dos gestores nas decis es de expans o internacional e na forma o de networks. Concluiu-se que o uso de networks vem demonstrando ser cada vez mais valioso para as PMEs, em especial em países em desenvolvimento como os países da América Latina.

Abstract: a study was conducted to compare the susceptibility of third instar larvae of lutzomyia longipalpis (l. & n.) (diptera: psychodidae: phlebotominae), the vector of the american visceral leishmaniasis to two strains of bacillus thuringiensis serovar israelensis, one strain of bacillus sphaericus (all pathogenic to diptera culicidae) and a strain of b. thuringiensis ser. morrisoni (pathogenic to larvae of anticarsia gemmatalis (hübner) (lepidoptera: noctuidae)). larvae of l. longipalpis showed similar susceptibility to the two strains of b. thruringiensis ser. israelensis, while b. sphaericus and b. thuringiensis ser. morrisoni showed low and no larvicidal effect to this vector, respectively.
