Magnetic resonance imaging with highresolution highly T2-weighted images is valuable in pre-operative localisation and characterisation of the defect, particularly if a transnasal endoscopic approach is planned. This report describes the radiological evaluation and surgical management of a 53-year-old man who presented with a spontaneous lateral sphenoid CSF fistula.
