%0 Journal Article %T Trial for the primary prevention of type II diabetes mellitus and the metabolic syndrome in La Reunion %A Favier F %A Fianu A %A Naty N %A Le Moullec N %J Revue M¨¦dicale de l'Assurance Maladie %D 2005 %I %X Aim: We performed a weight-reduction trial aimed at preventing type-II diabetes mellitus in a sample of high-risk patients aged 18-40 years in la R¨¦union. Method: In 2001-2002, we enrolled 439 high-risk patients through home screening in two low socio-economic districts of the city of St. Pierre. One of the districts served as the intervention group while the other was used as the control group. The prevention trial was based on community health methods, encouraging moderate regular physical activity and modifications in eating habits through workshops, discussion groups and peer education. A second assessment was made 16 months later. Results: The comparison before and after intervention versus control using an "intention to treat" analysis showed an average weight-loss of 1.2 kg ( p < 0.03) and a 1.8% ( p < 0.0001) reduction in the percentage of total body fat. In the "very high participation" subgroup of patients, the benefit was ever greater: average weight loss of 5.1 kg ( p < 0.0001), an average 4.2 cm reduction in waistsize and a 3.8 % ( p < 0.0001) lowering in total body fat. Almost 50 % of the participants in the intervention group regularly attended the workshops; more than half of the participants in the non-intervention district followed the life-style and dietary advice they received during the initial home screening.
