This collaborative work allows a fluently integration of the community pharmacist in the healthcare team, and improves communication between both health care professionals. Patient clinical record accessibility allows making patients interviews with more previous information (more complete). Interventions on possible drug-related problems suffered by the patient are easier. The Dader method have shown its efficacy on providing pharmacotherapy follow-up in community and hospital pharmacies.
