%X Primary antiphospholipid syndrome (APS) presents with a wide variety of neurological syndromes. But, hydrocephalus is seen very rarely. We report a case of gross communicating hydrocephalus in a 20-year-old woman with primary APS presented with headache and blurring of vision, without any focal neurologic signs. Magnetic resonance venography revealed superior sagittal sinus and bilateral cortical vein thrombosis.
