The patient had a laparotomy. There was a matted mass in the pancreatic region and lymph nodes compressing on the portal vein mimicking a pancreatic cancer (a). The patient had major surgery including distal pancreatectomy, splenectomy, removal of the lymph nodes at the porta hepatis, and suture ligation of the varices (b). Histopathology confirmed the diagnosis of abdominal tuberculosis. (Illustrated by Professor Fikri Abu-Zidan, Department of Surgery, College of Medicine and Health Sciences, UAE University).
