Maintenance of condylar position in a patient with mandibular deficiency

Abstract:

Maintaining the preoperative position of condyles within the temporal fossa is necessary in any orthognathic surgery, in order to ensure a stable postoperative result and the normal functioning of the temporomandibular joints. A variety of condylar positioning devices have been described to help preserve the preoperative position of condyles within the temporal fossa and prevent temporomandibular joint disorders or relapses.Case report: A 18-year-old female patient presented with a severe class II maxillofacial deformity, which was treated with sagittal split mandibular osteotomy and simultaneous genioplasty with the use of a polytetrafluoroethylene allograft. A condylar positioning device was used to maintain the preoperative position of the condyles. The patient did not develop any temporomandibular joint symptoms postoperatively, and the final skeletal result remained stable despite the considerable anterior displacement of the peripheral mandibular segment.
