Pharmacotherapy follow-up and the impact upon the glycaemic outcomes / Seguimiento farmacéutico y su impacto sobre los resultados glucémicos en pacientes diabéticos tipo 2 / Seguimento farmacêutico e o seu impacto sobre os resultados glicêmicos no tratamento de pacientes diabéticos tipo 2Keywords: Pharmacotherapy follow-up , Diabetes Mellitus , Glycaemic Control Abstract: The objective of this study was to analyze the impact of the pharmacotherapy follow-up in the Diabetes mellitus (DM), type 2 patients, through the evaluation of the clinical results. A prospective longitudinal study was carried out during six months, with 44 patients bearers of DM, type 2, who were enrolled in the attendance program of the public health service of Ribeir o Preto, SP, Brazil. The patients were divided in two groups, control and experimental (C and I, respectively). The changes in the clinical results of glycaemia in fast, post-prandial glycaemia and glycosylated hemoglobin of the two groups were appraised in the study. The results showed that, in the first three months, reduction of the glycaemia levels was observed, but only by the sixth month it showed to be statistically different (p=0,05) between groups C and I. In conclusion, the present work emphasizes the importance of the pharmacist interventions to the patients who are bearers of chronic diseases, as the Diabetes, with pharmacotherapy follow-up, for the improvement of health conditions.
