Abstract:

A esclerose lateral amiotrófica (ELA) é uma doen a neurodegenerativa progressiva com envolvimento do neur nio motor, podendo causar altera es na musculatura responsável pela degluti o. OBJETIVO: Avaliar a fase preparatória oral, oral e faríngea da degluti o em portadores de ELA utilizando a videoendoscopia da degluti o (VED) MéTODO: O desenho do estudo é do tipo coorte histórico com corte transversal em portadores de ELA submetidos à VED. Foram selecionados 11 pacientes (seis homens e cinco mulheres, com idade média de 61,7 anos) no período de janeiro a dezembro de 2011. RESULTADOS: Todos os pacientes apresentaram alguma fase da degluti o estudada alterada, porém, apenas 72,7% tinham queixa de disfagia. A fase preparatória oral mostrou-se alterada em 63,6% e a fase oral e faríngea em 100% dos indivíduos estudados, independente da consistência do alimento. A penetra o laríngea ou aspira o traqueal foi observada para o líquido em 90,9% dos pacientes durante a fase faríngea da degluti o. CONCLUS O: Mesmo na ausência de queixa, a disfagia é uma comorbidade frequente na ELA; as fases oral e faríngea s o as mais prevalentemente acometidas. A penetra o laríngea ou aspira o traqueal ocorrem mais durante a fase faríngea da degluti o com o líquido. Amyotrophic lateral sclerosis (ALS) is a progressive degenerative motor neuron disease that adversely affects the muscles responsible for swallowing. OBJECTIVE: To assess the oral preparatory, oral transit and pharyngeal phases of swallowing in ALS patients through endoscopic evaluation. METHOD: This cross-sectional historical cohort study included ALS patients submitted to endoscopic examination. Eleven patients (six males and five females; mean age of 61.7 years) were enrolled in the study from january to december of 2011. RESULTS: All patients had alterations in phases of the swallowing process, but only 72.7% complained of dysphagia. The oral preparatory phase was altered in 63.6% of the subjects; the oral transit and pharyngeal phases were altered in all studied individuals, regardless of food consistency. Laryngeal penetration or tracheal aspiration were seen in 90.9% of the patients during the pharyngeal phase while they were swallowing fluids. CONCLUSION: Even in the absence of complaints, dysphagia is a frequent comorbidity in ALS patients. The oral transit and pharyngeal phases were the most frequently affected. Laryngeal penetration or tracheal aspiration occurred more frequently during the pharyngeal phase while patients were swallowing fluids.
