A 28-year-old man presented to the department of periodontology with chief complaint of recurrent swelling with pus discharge in the vestibular area of maxillary right lateral incisor since 3 weeks. On clinical examination a periodontal pocket of 12 mm was noted on palatal aspect in middle third running towards apex. On radiographic interpretation an irregular radiolucency at apex gave a suspicion of palatoradicular groove, which was then explored and managed surgically along with bone graft
