Abstract:

Chronic kidney disease (CKD) is a nephrological syndrome characterized by chronic, progressive and non-reversible nephron loss due to various diseases. When the CKD-EPI eGFR rate falls below 45 ml/min, patients should be closely monitored by a multidisciplinary team in the nephrology low clearance out-patients clinics. The core members of this team are nephrologists and nurses involved in patient training and education. Education nurses are the most convenient staff for communicating with patients easily. The duties of the education nurse include helping the patients and their families to adjust to lifestyle changes, preventing discrepancies, and helping patients and their families to cope with the problems they face, and helping them to adapt in the disease process and return in their daily life. Changes in the patient's condition can be detected in the early stages and early intervention is provided. Patients can take responsibility for their treatment and care by increasing their satisfaction, In this case, low clearance out-patients ?nursing educational process is examined according to the nursing diagnoses developed by the North American Nursing Diagnosis Association (NANDA) and ?Functional Health Pattern Model which is developed by Gordon in 1982
