Abstract:

目的:探讨基于超声无创全身动脉僵硬度自动测量系统(Automatic Measurement of Arterial Stiffness,AMAS)测得的颈动脉–股动脉脉搏波速度(carotid-femoral pulse wave velocity, cfPWV)在评估高血压患者动脉僵硬度的临床应用。方法:回顾性分析2020年8月至2022年1月于本院健康管理中心体检的60例高血压患者为高血压组和60例健康体检者作为对照组。根据血压水平分级标准将高血压组分为2组,1级(轻度)为第1组,2级(中度)和3级(重度)为第2组。应用同一超声诊断机AMAS测量受试者的cfPWV。比较各组cfPWV、收缩压、舒张压等参数。结果:cfPWV在三组之间的差异有统计学意义(P < 0.05),cfPWV 值随血压增高而增大,且组间两两比较差异均有统计学意义(P < 0.05)。年龄和收缩压是cfPWV的影响因素,且年龄和收缩压与cfPWV呈正相关。结论:通过AMAS检测cfPWV有助于早期发现高血压患者动脉僵硬度,对临床早期干预以及预防心脑血管并发症具有重要意义。
Objective: To investigate the clinical application of carotid femoral pulse wave velocity (cfPWV) measured by ultrasonic non-invasive automatic measurement of arterial stiffness (AMAS) in evalu-ating arterial stiffness in patients with hypertension. Methods: From August 2020 to January 2022, 60 patients with hypertension as the hypertension group and 60 healthy people as the control group were selected from the health management center of our hospital. According to the classifica-tion standard of blood pressure level, the hypertension group was divided into two groups, grade 1 (mild) was the first group, grade 2 (moderate) and grade 3 (severe) was the second group. The same ultrasound diagnostic machine AMAS was used to measure the cfPWV of subjects. The parameters such as cfPWV, systolic blood pressure and diastolic blood pressure were compared. Results: The difference of cfPWV among the three groups was statistically significant (P < 0.05). The value of cfPWV increased with the increase of blood pressure, and the difference between the two groups was statistically significant (P < 0.05). Age and systolic blood pressure are the influencing factors of cfPWV, and age and systolic blood pressure are positively correlated with cfPWV. Conclusion: The detection of cfPWV by AMAS is helpful for early detection of arterial stiffness in patients with hy-pertension, and is of great significance for early clinical intervention and prevention of cardiovas-cular and cerebrovascular complications.
