Abstract: objective: report a case of ovarian primary leiomyoma. ovarian leiomyomas are rare ovarian lesions. case: the patient is a 46 years old female, who underwent sonographic pelvic examination for for right adnexal mass control. a right ovarian mass was discovered, that measured 45 x 36 mm. the appearance was homogeneous, and it was described solid and encapsulated. histerectomy and bilateral oophorectomy was performed. histopathologic study was consistent with a primary ovarian leiomyioma. immunohistochemical results were inhibina and s-100 negative and smoot muscle actin and vimentin was positive, consistent also with a primary ovarian leiomyoma. discussion: these tumors are frequently underdiagnosed due their rarity. they are benign with a survival rate 0f 100% at five years
