Abstract: in this paper, is presented the case of a 32 year old, righ-handed male patient, with a clinical picture characterized by facial pain and diplopia 12 months before admission. the patient was then a worker in the united states of america and received medical attention there. after performing a ct sean, no definite diagnosis could be established. the patient returned to mexico and after 6 months, developed a progressive paralysis of the oculomotor nerve. a new neurological evaluation showed the existence of a tumorallesion on the left cavernous sinus region with perilesional edema. the diagnosis was a meningioma of the lateral wall of the cavernous sinus. the patient underwent surgery through a dolenc approach with a total resection of the tumor. the histopathological diagnosis was tuberculoma. the patient received treatment with ryfampicin and ethambutol for the next six months. after surgery the patient recovered from the iii nerve paralysis and remains asymptomatic after 5 years of follow up. a brief analysis about this uncommon presentation of tuberculomas is made with a review of the literature.
