Adapting of the Dader Method for pharmacotherapy follow-up to primary care level / Adaptación del Método Dáder de seguimiento farmacoterapéutico al nivel asistencial de atención primariaKeywords: Community pharmacy services , Pharmacotherapy follow-up , Primary care , Dader method Abstract: Providing pharmacotherapy follow-up together the pharmacist and the family physician in a health care clinic, produces some advantages on identifying drug-related problems. This collaborative work allows a fluently integration of the community pharmacist in the healthcare team, and improves communication between both health care professionals. Patient clinical record accessibility allows making patients interviews with more previous information (more complete). Interventions on possible drug-related problems suffered by the patient are easier. The Dader method have shown its efficacy on providing pharmacotherapy follow-up in community and hospital pharmacies. This paper propose the adaptations needed for applying the Dader method in a primary care level.
